Jonathan started his role as the Director of Strategic Development at Yeovil District Hospital NHS Foundation Trust in June 2015. Previous to this Jonathan was the Director of Urgent Care and Long Term Conditions and Director of Operations at the Trust. Jonathan graduated from the University of Reading in 1993 and has 16 years experience working in a variety of roles in both hospitals and service planning across the NHS in the South West and South East of England.
As part of the South Somerset Symphony Programme Yeovil District Hospital NHS Foundation Trust in partnership with Somerset CCG, South Somerset Healthcare GP Federation and Somerset County Council are working together to deliver an Integrated Primary and Acute Care System as one of the nine national Integrated primary and acute care systems Vanguard sites. The Trust and GP Federation are joining together to establish a joint venture which will hold a single budget for the population and target resources to parts of the system where they can make the most difference to patients. Work has initially focused on implementing a new care model for approximately 1,500 South Somerset residents who have multiple long term conditions.
