Anna is the Communications Manager for Home From Home Care, a specialist residential care provider for adults (18+) with learning disabilities and complex needs. Having worked with this unique family-led organisation for 11 years, Anna is passionate about sharing her knowledge & insights from such a dynamic organisation, where their innovative, transformative and data informed NewModel of care changes lives and delivers meaningful outcomes for young people with complex needs. This NewModel uses real-time data to anticipate, adapt and respond to an individual’s behaviours, goals and preferences, supporting colleagues with specialist resource teams, so the individual has the right support for their changing needs and creates ‘The World on My Terms’.
