Open Forum Events are delighted to be hosting the NHS 5YFV Vanguard: New Care Models – Leading the Way conference. This is the fourth in a highly successful series of events focusing on the Five Year Forward View.
Published in 2014 and formulated by a coalition of the leading NHS organisations, the Five Year Forward View represents a ‘road map’ for health services over the next five years. The establishment of vanguard sites signals one of the first steps in the implementation of the new care models that are described within the 5YFV.
The aim of initiating these new models of working is to organise services that focus on the needs of patients and look to dismantle the traditional boundaries as to where, when and how care is delivered.

The agenda at NHS 5YFV Vanguard: New Care Models – Leading the Way is designed to provide delegates with a detailed insight as to how the NHS may operate into the future by adopting these new modes of working. We will hear directly from the vanguard sites on how their particular model can and is working in practice; the outcomes that can be achieved and the challenges to be overcome. Delegates will gain an understanding as to how their own organisations may be transformed and how their professional practice may be influenced.

The establishment of vanguard sites is the first stage of implementing the Five Year Forward View with the aim to develop new models of working to improve outcomes, patient experience and value for money. Breaking down the traditional boundaries between GPs and secondary care, between physical and mental health, between health and social care are seen as the way forward to a more locally delivered integrated and collaborative service.
